New Patient Information
What to Bring With You to Your Appointment:

Your Driver's License

Your Most Current Insurance Card

An Up-To-Date List of Your Medications (including frequency and dosage)

An Up-To-Date Surgical History

Completed, Signed and Dated Forms (If you choose to fill them out in our office please arrive 30 minutes before your appointment)

If Applicable: Any Images, X-rays, Discs, Results, etc.

Please arrive 15 minutes prior to your scheduled appointment time

Please check with your insurance carrier to see if a referral to a specialist is required, if so please contact your primary care provider to have the referral placed prior to your appointment

Co-pay is expected at time of service
If you need to cancel or reschedule your appointment, please give us 24 HOURS NOTICE by calling our office at 330-856-1700.
Please be aware that if you are 15 MINUTES LATE to your appointment, you will have to be rescheduled, due to the time requirements that allow a thorough and comprehensive examination involved with a new patient appointment.
